My tutorial for simplified book folding http://www.diypics.com/simplified-folded-book-art/ has produced a lot of questions. This video is an effort to answer most questions about book folding art.
Also, I now have pre-striped pages in the article link to above that you can use for making a pattern if you don't have Photoshop. About half-way down the article you'll see links to several templates for different sized books.
